If it has this bolt pattern you should be good to go. The bolt pattern looks just like this in the link.

I'm sure you can find the bolt kit online. Or maybe the CDC uses the same which by eye it looks the same.
Looks like you have all the parts other than the gasket. Double check the box, mine came with another gasket same as the original. Yes, the bracket is slightly twisted, looks like it is correct, and mounts to the last bolt on the intake manifold
I'll take a pic of the gasket and bracket location when I return over the weekend.
